Driver Information Exchange
In most cases, the reporting officer will provide all involved drivers with a Driver Information Exchange form either at the scene of the crash or shortly afterward via email.
If you did not receive this information—or if it was misplaced—you may download a free copy through LexisNexis® Police Reports by registering for an account.
Crash Report

Illinois Motorist Report
As of August 20, 2021, Public Act 102-1056 repealed portions of the Illinois Vehicle Code that previously required drivers involved in certain types of accidents to file a separate report with the Illinois Department of Transportation.

2. Crash Report
- Purchase your official crash report online through LexisNexis® Police Reports.
